The salary range for this position is $42.50 - $63.12 (Hourly Rate)
Placement within the salary range is dependent on several factors such as relevant work experience and internal equity. **This position is located in Chicago, IL and does require travel throughout the Chicagoland area for outreach, to dialysis units, and to satellite clinics.**

Northwestern Medicine’s Comprehensive Transplant Center has committed to a campaign to eliminate transplant disparities for residents of the greater Chicagoland area in relation to transplant care for kidney and liver disease. We are dedicated to the development of a deeper understanding of the historical barriers and cultural concerns of the community, and strive to work together with the community to overcome those obstacles. Through our program, we aim to increase awareness, improve access, and provide excellent outcomes through outreach; community based participatory research, and patient and provider education. In this role, the selected candidate would travel with the kidney transplant team to community satellite clinics throughout the Chicago metro area and would be the lead nurse in the kidney transplant evaluation within the satellite clinics. This role would also include outreach to dialysis units to educate patients and professionals regarding kidney transplant options. This position is a mix of clinical care and outreach. This position also includes much travel throughout the different Chicago neighborhoods along with the Greater Chicagoland area.

The Transplant Nurse Coordinator reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.
The Transplant Nurse Coordinator is a Registered Nurse with experience in and knowledge of transplantation and living solid organ donation. Practicing under the supervision of transplant surgeons and physicians, the Transplant Nurse Coordinator ensures coordination of the clinical aspects of transplant patient and living donor care throughout all phases of care. The job duties vary with the particular setting or assignment. Generally, Transplant Nurse Coordinators are assigned to kidney and pancreas intake, liver and small bowel intake, or post-transplant. Transplant Nurse Coordinators may also be designated as the Independent Living Donor Advocate.
