How you will make an impact:
As a MDS Coordinator at Mountain Vista Senior Living, you will be responsible for coordinating and overseeing the full collaborative, interdisciplinary assessment and care planning process of the MDS. Will ensure that all contributing documentation and information reported on the MDS is accurate, timely and submitted according to state/federal regulations and facility policy and procedure. Encourages each resident's ability to maintain or attain the highest practical physical, mental, and psychosocial well-being, through Community programs and procedures in compliance with applicable laws and regulations, ABHM mission and core values.
Schedule: This is a full-time position, Monday - Friday, day shift.
